Our Eastside clinic is a full-service health care resource for all age groups, including pediatric patients. Providers at the Eastside clinic offer their patients a wide range of services including physical check-ups, immunizations, health screenings, treatment for various ailments, health care advice and education, pharmacy assistance programs, and linkage with community resources.
The Braddock Endowment Fund helps eliminate financial barriers to care by providing patients with low-cost prescription vouchers and free public transportation passes. CHFM will continue to work closely with community partners to help improve the quality of care and resources available in order to better address the needs of east Gainesville.

Our Haile Plantation clinic provides a wide variety of services for patients ranging from children to elders, including immunizations, treatment of chronic and acute illnesses, minor surgery, gynecology, dermatology, health maintenance, cryosurgery, and skin care. Some conditions treated include diabetes, high blood pressure, coronary artery disease, osteoporosis, asthma, high cholesterol, and thyroid problems.

Our Jonesville clinic offers comprehensive care to patients ranging from children to elders. The providers at Jonesville offer a wide variety of services, including treatment of acute and chronic illnesses, minor surgeries including vasectomies, gynecology, dermatology, health maintenance, cryosurgery, colposcopy, skin care, immunizations, and procedures such as placements of IUDs and nexplanons. Providers at the Jonesville clinic are happy to serve not only the Gainesville and Jonesville communities, but also surrounding areas such as Newberry, Archer, Williston, Bronson, High Springs, Fort White, Alachua, Lake City, and Ocala.

Our Main Street clinic offers general medical care for all age groups, including pediatric patients. The clinic offers multiple screenings, such as those for cancer, heart disease, cholesterol, blood pressure and diabetes. Patients can also receive health maintenance checkups, immunizations, school physicals, general gynecology services, family planning, and menopause care. This clinic also houses our Family Medicine Residency Program.

Old Town is located approximately 45 miles west of Gainesville very near the Suwannee River and only miles to the Gulf of Mexico. The providers at Old Town offer access to high quality, individualized healthcare in a rural health setting. Our practice is focused on earning our patients’ trust through providing compassionate, skilled care and education while also empowering our patients to partner with our healthcare team for positive health outcomes.

Our Springhill clinic offers comprehensive care for patients ranging from adolescents to elders. The three-story building is home to multiple specialties and services to better care for the local community, and we are excited to accommodate a variety of medical needs without the burden of traveling long distances.

Our Lake City clinic offers comprehensive care for patients ranging from children to geriatrics. Besides routine and preventive health care, our providers offer many services including: minor skin/ laceration repairs, sports physicals, yearly exams, treatment of diabetes mellitus, hypertension, and coronary artery disease. We recently added women’s health options including IUD and Nexplanon insertion & removal.

Our Heath Brook clinic in Ocala provides preventive healthcare and general medical treatment, including flu shots, care for a sore throat, gynecological services, physical check-ups, nutrition counseling, immunizations, and ongoing support managing chronic conditions such as asthma and diabetes. Our providers, as well as a full team of nurses and other healthcare professionals, provide all the resources and support you need to improve your health and implement healthy practices at home.

Our Starke clinic’s primary mission is to provide the community and surrounding areas access to high quality individualized healthcare in a rural health setting. Our clinic provides comprehensive care to patients ranging from neonate to geriatrics. Our Starke clinic also offers an on-site UF Health Pathology Lab.

The UF SHCC has been meeting the health care needs of UF students since 1906, and recently moved into a brand new, state of the art WELL-certified building next to the O’Connell Center. Our mission is to help every student achieve optimal health in the pursuit of personal and academic success. Services provided include general medical, sports medicine and acute care, concussion care, women’s health, sexual health, psychiatric services, physical therapy, on-site laboratory, radiology and ECG services, and a full-service pharmacy.
