Please provide information as requested below for the July 1, 2018 – June 30, 2019 evaluation period.

The administrative office will obtain Family Medicine and Ambulatory Care Clerkship, Family Medicine Residency Program, Common and Advanced Procedure evaluations.
  • Directions: Please provide a concise description of your patient care activities emphasizing accomplishments you wish to highlight. These may include commitment to ongoing growth in clinical performance, quality of care activities, clinical committee service, reviewer of clinical journals, clinical scholarship (publications, presentations), and awards or honors.
  • Directions: Please list grant and research activities.
  • Directions: Please provide your contributions to the discipline, community membership and service.
  • Directions: Please provide three goals you would like to accomplish in the upcoming year.
  • Directions: Please provide a concise description of additional activities and accomplishments you wish to highlight.
